Name Change

Do you need to change your name in your Passport?

If your passport was issued less than 1 year ago and your name has been legally changed due to marriage or court order.

  1. Assemble your application:
    • Provide your current, valid U.S. passport.
    • Complete form DS-5504: Application for a U.S. Passport: Name Change, Data Correction, and Limited Passport Book Replacement.
    • Provide the original marriage certificate or the government-issued document evidencing your legal name change.
    • Provide one (1) identical photo 2×2 inches. For exact specifications on the photo, please click here.
    • No fee is required.
  2. To submit the application, request an appointment at the Department of State Website. Each applicant requires his/her own appointment.
  3. Allow approximately two weeks for processing. Passports must be collected at the ACS section by the applicant or authorized representative (with a signed letter from the applicant). An appointment is not necessary to pick up your passport.

If your passport was issued more than 1 year ago and your name has been legally changed due to marriage or court order.

  1. Assemble your application:
    • Provide your current, valid U.S. passport
    •  Completed Form DS-82, Application for a U.S. Passport by Mail.
    • Provide the original marriage certificate or the government-issued document evidencing your legal name change.
    • Provide one (1) identical photo 2×2 inches. For exact specifications on the photo, please click here.
    • The fee is $110 payable in U.S. dollars or the equivalent in Kenyan Shillings, or credit card (VISA, MasterCard, Dinners, American Express, and Discover).
