Passport Replacement – Lost and Stolen

Do you need to replace a Lost or Stolen passport?

Adult Passport Replacement

  1. Assemble your application:
    • Complete Form DS-11. Enter your information online and print Form DS-11 OR print out the blank form and then complete using black ink on one-sided pages. Failure to provide information requested on this form, including your social security number, may result in significant processing delays and/or the denial of your application. The form can also be obtained from American Citizen Services at the U.S. embassy.
    • Complete Form DS-64: Statement Regarding a Lost or Stolen Passport. Complete and print the form.
    • Applicants should present documents that prove U.S. citizenship (birth certificate, naturalization certificate, old passports) and identity, such as driver’s license.
    • Bring one (1) identical photo 2×2 inches. For exact specifications on the photo, please click here.
    • Obtain a police abstract. The adult must report the loss or theft of his/her passport to a police station close to where the loss took place. Our office requires this abstract in most circumstances.
    • Travel itinerary – If the passport bearer is traveling out of the country soon and is applying for an emergency passport, please bring a copy of the travel itinerary.
    • The fee is $145, payable in U.S. dollars or the equivalent in Kenyan Shillings, or by credit card (VISA, MasterCard, Dinners, American Express, and Discover).
  2. To submit the application, request an appointment at the Department of State Website. Be sure to schedule a separate appointment for each individual applying.
  3. Allow approximately two weeks for processing. Passports must be collected at the ACS section by the applicant or authorized representative. An appointment is not necessary to pick up your passport.

Minor Passport Replacement

  1. Assemble your application:
    •  Complete Form DS-11. Enter your information online and print Form DS-11 OR print out the blank form and then complete using black ink on one-sided pages. Failure to provide information requested on this form, including your social security number, may result in significant processing delays and/or the denial of your application. The form can also be obtained from American Citizen Services at the U.S. embassy. Do not sign Form DS-11 on page 1.
    • Complete Form DS-64 (PDF 49.6 KB) Statement Regarding a Lost or Stolen Passport.  Complete and print the form.
    • Bring the birth certificate of the child/children.
    • Bring one (1) identical photo 2×2 inches. For exact specifications on the photo, please click here.
    • The fee is $105 payable in U.S. dollars or the equivalent in Kenyan Shillings,  or by credit card (VISA, MasterCard, Dinners, American Express, and Discover).

Note: Both parents must be present with the child and must carry their passports or any other form of identification. If one or both parents are out of the country, the absent parent(s) should provide a notarized copy of statement of consent (DS-3053) (PDF 41.2 KB) and certified copy of valid photo ID. Scanned and faxed copies will not be accepted.

If the applying parent has sole custody, that parent will be required to provide court documents to prove this in order to apply for their child’s passport alone.

If you are the applying parent or guardian and the written consent of the non-applying parent or guardian or evidence of sole custody cannot be obtained in a timely manner, complete the DS-5525 “Statement of Exigent/Special Family Circumstances” (PDF 40.9 KB). The statement must explain in detail the non-applying parent’s or guardian’s unavailability and recent efforts made to contact the non-applying parent. You may also be required to provide evidence (e.g., custody order, incarceration order, restraining order) to document your claim of exigent or special circumstances. Please submit the DS-5525 with the DS-11 on the appointment day.
